Introduction: Pressure ulcers are a nursing-sensitive outcome, especially relevant in intensive care units due to the high risk faced by critically ill patients. Objectives: To describe a structured prevention and intervention program to improve the quality of nursing care provided to critically ill patients at risk of developing pressure ulcers. Material and Methods: The four-phase Plan-Do-Study-Act methodology was used to structure a continuous quality improvement project consisting of five main activities: Needs Assessment; Literature Review; Continuous Training; Document Development; and Definition of Performance Indicators. Results: The planning resulted in the creation of a comprehensive, evidence-based program addressing various dimensions of pressure ulcer prevention in intensive care units. Conclusions: This project establishes a solid foundation for the prevention of pressure ulcers in intensive care units. The developed plan has the potential to improve the quality of care and provide a framework for continuous evaluation. The implementation of the subsequent phases (Do, Study, Act) will be crucial for the project's success and sustainability
